Children's Extended Health Plan BenefitsChildren's Health Benefit Plan provides healthcare benefits for students belonging to the lower-income category. It is for students up to 18 who study in high school.
This program provides coverage for:

  • Emergency ambulance services( Emergency ambulance trip to the nearby hospital)
  • dental care( provision of basic services like x-rays and teeth cleaning)
  • essential over the counter medications
  • necessary diabetic supplies (Availability of injections, lancets, and testing strips)
  • prescription drugs( Over the counter products such as children vitamins)
  • eyeglasses( Provision of eyeglasses every year)

 

Who are the eligible applicants for Children Health Benefit Plans:

To meet the eligibility criteria of this plan, you and your family members must be

  • A resident of Alberta
  • Have the status of a permanent resident or Canadian citizen
  • Not receiving any health benefits from other government programs:
    1. Income Support
    2. Child and Youth Support Program
    3. Assured Income for Severely Handicapped
    4. Canadian government programs for Albertans with First Nations or Inuit status.

To avail the benefits, your income must fall below these maximum guidelines based on family members: 

  • 1 adult+ 1 child =$26,023
  • 1 adult + 2 children= $31,010
  • 1 adult + 3 children=$36,325
  • 1 adult + 4 children= $41,957
  • Couple with no child=$23, 212
  • Couple+1 child=$31,237
  • Couple+2 children=$36,634
  • Couple+3 children=$41,594
  • Couple+4 children=$46,932

 

Procedure to apply :

  • Fill up the application form
  • Sign and date the sections marked with My declaration and consent for Canada Revenue Agency.

Child Health Benefits Card:

After the approval, you will receive a health benefits card in your mail listing the child's name and identification number. Each time your child gets any healthcare services, it needs to be shown to the doctor, dental provider, pharmacist, optician and ambulance assistant.

 

Annual evaluation:

Every June, Canada Revenue Agency keeps a check that your income is under our determined levels as mentioned above. If it meets the criteria, you are automatically enrolled for the following year.

 

Health Benefits Exception Committee:

This committee is mainly established to review the request for drugs, ambulance, dental, and optical services that are not covered under the benefits list.
Suppose your appeal is for drugs or medicines that are not covered under the benefits list. In that case, your doctor will provide a detailed medical rationale using the Request for Prescription Drugs and Nutritional Products Form.
If your appeal is for optical services that are not covered under the benefits list, please have your optical service provider complete the Request for Optical Services form.
